About Your Visit

Insurance Information

We accept most major insurance plans.

By law, we are providers for all workers' compensation medical benefits, whether or not part of an employer's list of physicians. We accept assignments from Medicare. As specialists providing unique care, most plans allow patients to see us out-of-network. Payments are the responsibility of the patient and are due at the time of service.

Appointments

Please call our main phone line at (602) 953-9500 to schedule an appointment with any of our three offices, Scottsdale, Westside or East Valley. If you are unable to keep your appointment, please notify us 24 hours in advance, and we can schedule a new appointment for you at that time. There are occasions when our normal office schedule may be altered, because our doctors are delayed at the hospital, or an unexpected emergency arises. Your understanding and cooperation is appreciated.

Fees & Payments

We make every effort to decrease the cost of your medical care. Therefore, we request payment arrangements for all office services at the time they are rendered unless prior arrangements have been made. We accept cash, checks, MasterCard and Visa for your convenience. If we are a participating provider of your insurance company, we will bill them. However, payment is the patient’s responsibility. We will help in any way we can to assist you in handling claims.

Prescriptions & Renewals

Regular prescription refills (even if no refills are available) should be requested directly from your pharmacy; please allow up to 72 hours for your request to be processed. It is the patient’s responsibility to check with their pharmacy to see if the prescription is available for pick up. In addition, due to the high volume of prescription requests we receive; please contact your pharmacy at least one week prior to your prescription running out.

You must have an office visit within a minimum 3 month time period, (sometimes more frequently if your doctor requires) to continue to receive refills including maintenance medications.

New prescriptions for medication we have not prescribed, or change in medication, will require an office visit.

Refill requests will not be filled after hours or on the weekend.

For Your Appointment

If you are a new patient, please arrive 30 minutes early to your appointment in order to complete the registration process. In addition, please bring the following:

  • Driver's License or State ID
  • Insurance Information
  • Copies of Operation Records, Medical Records, X-rays, MRIs, and CT Scans from Prior Doctor Visits
  • A List of Current Medications and Any Known Allergies
